Thinking of Travelling?

Our experienced Practice Nurses can offer travel advice and vaccinations.

How to book an appointment?

    1. Complete a travel form for each person planning to travel. (These can be collected from either our East Bergholt or Capel St Mary site, or downloaded from the Practice website). Online Travel Form
    2. Hand your completed travel forms into reception who will book your travel appointment with a Practice Nurse. (Longer appointment times will be required for larger groups travelling together).
    3. Prior to your appointment your allocated Nurse will review your travel forms and determine whether any vaccinations are required. (If the Nurse can see from your medical records that you have either had the vaccinations advised, or the country you are visiting does not require any vaccinations but ONLY travel advice, you will be contacted prior to your appointment and any adjustments to your appointment made).
  • At your appointment you will be given the appropriate safe travel advice and vaccinations! – Please be aware some vaccinations incur a fee.

IMPORTANT - Please ensure you fill in your travel forms and book your appointment at least 10-12 weeks prior to travelling. Our Practice Nurses get booked up in advance and may need to order vaccine stock in ready for your appointment!
